Om beleid te kunnen ontwikkelen voor het terugdringen van de verkeersonveiligheid moet bekend zijn hoe groot het probleem is en waar de grootste problemen zich voordoen. Informatie over het aantal verkeersongevallen en -slachtoffers is dus zeer belangrijk. Informatie over de kenmerken van deze ongevallen en slachtoffers is echter nog belangrijker. De meest relevante kenmerken moeten komen uit het Bestand geRegistreerde Ongevallen in Nederland (BRON). Dit bestand komt tot stand op basis van gegevens die de politie verzamelt en aan de Dienst Verkeer en Scheepvaart (DVS) van Rijkswaterstaat aanlevert. Het is echter bekend dat er verkeersongevallen zijn die niet in BRON geregistreerd worden. Om toch een goed beeld te krijgen van de verkeersveiligheidssituatie in Nederland worden twee belangrijke indicatoren, het aantal verkeersdoden en het aantal ernstig verkeersgewonden, niet bepaald op basis van BRON alleen. Het aantal ernstig verkeersgewonden wordt ieder jaar geschat door de SWOV en daarvoor wordt naast BRON ook de Landelijke Medische Registratie (LMR) gebruikt. Het aantal verkeersdoden in Nederland wordt sinds 1996 jaarlijks vastgesteld door het Centraal Bureau voor de Statistiek (CBS), in samenwerking met het ministerie van Infrastructuur en Milieu (IenM). Dit gebeurt op basis van drie bronnen: de zogeheten B-verklaringen (doodsoorzaakverklaringen die in principe bij elk sterfgeval naar het CBS gestuurd worden), verslagen van de arrondissementsparketten en BRON. Aanleiding en doelstelling onderzoek Op 18 april jl. maakte de minister van IenM het door het CBS bepaalde aantal verkeersdoden in 2010 bekend: 640. Daarmee is het aantal verkeersdoden gedaald met 11% ten opzichte van 2009. Daarnaast is de registratiegraad van verkeersdoden in BRON met 84% veel lager dan de jaren ervoor. Dit vormde voor de SWOV aanleiding om zich in de kwaliteit van de registratie van verkeersdoden in Nederland te verdiepen. Allereerst is het doel te achterhalen welke oorzaken er zijn voor het teruglopen van de registratiegraad van BRON in 2010. Daarnaast willen we vaststellen of het aannemelijk is dat er verkeersdoden gemist worden, met andere woorden of het aantal verkeersdoden in Nederland in werkelijkheid anders is dan het CBS heeft bepaald. Bevindingen ten aanzien van de bruikbaarheid van de politieregistratie voor BRON Een belangrijke ontwikkeling in 2010 is het afschaffen van de zogeheten registratieset bij het registreren van ongevallen door de politie. Dit formulier bevatte altijd uitgebreide informatie over ongevallen. De politie maakt sinds het vervallen van de registratiesets vaker gebruik van kenmerkenmeldingen, die echter niet de informatie bevatten die nodig is om verkeersveiligheidsbeleid of -onderzoek op te baseren. Een andere belangrijke ontwikkeling was de invoering in 2009 van één landelijk informatiesysteem (Basisvoorziening Handhaving of BVH) voor de politie voor het registreren van meldingen, het verwerken van aangiftes en het afhandelen van incidenten. De Algemene Rekenkamer trekt in 2010 de sombere conclusie dat de invoering van BVH een negatief effect kan hebben op de kwaliteit van de ongevallenregistratie. Het was voor ons niet mogelijk om hard aan te tonen dat bovengenoemde twee ontwikkelingen de oorzaak vormen voor de plotselinge afname van het aandeel verkeersdoden dat in BRON geregistreerd staat. Er zijn in dit onderzoek echter voldoende aanwijzingen gevonden om te concluderen dat die samenhang wel bestaat. Overigens wordt bij elk niet-natuurlijk overlijden in Nederland de politie ingeschakeld. Dit betekent dat de in BRON ontbrekende verkeersdoden wel bij de politie bekend zijn, maar wellicht niet noodzakelijkerwijs als slachtoffer van een verkeersongeval. Gegevens over deze doden worden dus niet naar DVS verzonden. Bevindingen ten aanzien van het aantal verkeersdoden De verkeersdodenstatistiek van het CBS is slechts in beperkte mate gebaseerd op BRON. De andere twee bronnen die aan de verkeersdodenstatistiek ten grondslag liggen (de B-verklaringen en de verslagen van de arrondissementsparketten) zijn onafhankelijk van BRON. Het teruglopen van de registratiegraad van verkeersdoden in BRON tot 84% hoeft dus geen gevolgen te hebben voor de registratie van verkeersdoden in een van de andere twee gebruikte bronnen. Wanneer een verkeersdode in ten minste een van de drie bronnen geregistreerd is, komt deze in de verkeersdodenstatistiek van het CBS terecht. Op basis van de informatie die het CBS heeft verstrekt, concluderen wij dat het CBS op basis van de beschikbare gegevens op correcte wijze het jaarlijks aantal verkeersdoden in Nederland vaststelt. Wat betreft de volledigheid van de gegevens waarop het CBS zich baseert, concluderen wij dat het zeer waarschijnlijk is dat er doden zijn waarvan wordt aangenomen dat zij een natuurlijke dood zijn gestorven, terwijl zij in werkelijkheid zijn overleden ten gevolge van een verkeersongeval. Deze onderregistratie ligt naar ons inzicht bij de behandelend arts die moet bepalen of iemand al dan niet een natuurlijke dood is gestorven. Uit de literatuur weten we dat er in deze fase van het registratieproces het een en ander mis gaat. Hoeveel verkeersdoden hierdoor gemist worden, is zonder nader onderzoek niet vast te stellen, noch of het om een specifieke deelgroep van de verkeersdoden gaat. Ook is vooralsnog onbekend of dit nu vaker, minder vaak of even vaak voorkomt in vergelijking met een aantal jaar geleden. Bij de vaststelling van het aantal verkeersdoden maakt het CBS (nog) geen gebruik van de LMR. Dat bestand bevat echter gegevens over uit Nederlandse ziekenhuizen ontslagen patiënten (waarbij overlijden een vorm van ontslag is) en kan dus een aanvulling zijn op de drie bronnen die het CBS al gebruikt. Daarbij moet met de volgende zaken rekening worden gehouden: - In de LMR hoeft het overlijden niet het gevolg te zijn van het genoemde verkeersongeval. - In de LMR wordt bij een verkeersongeval waarbij geen motorvoertuig betrokken is niet de eis gesteld dat dit op de openbare weg heeft plaatsgevonden. Daardoor zijn verkeersongevallen in de LMR dus niet noodzakelijk verkeersongevallen volgens de formele definitie. Om na te gaan hoe de LMR ondanks deze nadelen toegevoegde waarde kan hebben bij het bepalen van de doodsoorzaak, zijn de LMR-bestanden van de periode 2007-2009 geanalyseerd; de LMR voor 2010 was ten tijde van dit onderzoek helaas nog niet beschikbaar. Op jaarbasis zijn er in deze bestanden circa 65 doden die een verkeersongeval hebben gehad, maar niet in BRON terug te vinden zijn. Het CBS is op verzoek van de SWOV nagegaan of deze personen bekend zijn in hun verkeersdodenstatistiek. Het CBS vond ongeveer de helft van deze doden in de verkeersdodenstatistiek terug. Van de andere helft (circa 35 doden) oordeelde het CBS dat er sprake was van een andere doodsoorzaak. Voor het overgrote deel vonden zij voor deze overledenen in de overige bronnen aanwijzingen dat het overlijden niet het gevolg zou zijn geweest van een verkeersongeval. Van vijf doden per jaar voor wie de LMR melding maakt van een verkeersongeval, vond het CBS in de overige bronnen géén aanwijzingen dat het niet zou gaan om een verkeersongeval. Deze overledenen worden dus mogelijk gemist bij het bepalen van de werkelijke omvang van het aantal verkeersdoden. Omdat de LMR-gegevens over 2010 nog niet beschikbaar waren ten tijde van dit onderzoek, kon niet nagegaan worden of bovenstaande bevindingen ook gelden voor 2010. Met andere woorden, het is nog niet bekend of er in 2010 ook hooguit vijf overleden personen zijn die worden gemist. Aanbevelingen Er is niet goed in te zien waarom de registratiegraad van verkeersdoden in BRON niet 100% zou kunnen zijn. Ongevallen waarbij zeer ernstig letsel wordt opgelopen en doden vallen, zijn zeer ernstige gebeurtenissen waarvan de politie op de hoogte is of zou moeten zijn. De extra administratieve belasting van het opmaken van processen-verbaal of registratiesets van verkeersongevallen door de politie kan naar verwachting beperkt zijn, zeker in het geval van verkeersdoden. Deze zijn namelijk al bekend bij de politie, aangezien bij elk niet-natuurlijk overlijden de politie wordt ingelicht. Aanbevolen wordt dat deze slachtoffers allemaal in BRON geregistreerd worden. De SWOV geeft in overweging de registratieset voor verkeersdoden en ernstig verkeersgewonden opnieuw in te voeren. Een andere aanbeveling is om de kenmerkenmelding uit te breiden met meer variabelen, zodat het eenvoudiger is informatie uit andere bronnen hieraan te koppelen. De noodzakelijke verbetering van de registratie door de politie betekent dat er allereerst nieuwe aanwijzingen aan de politie moeten worden gegeven. Vervolgens moet de politieorganisatie in de gelegenheid worden gesteld om de afspraken na te komen. Dat houdt onder meer in dat de software die de organisatie gebruikt gebruiksvriendelijker moet worden. Dit leidt tot meer efficiëntie en een hogere kwaliteit. Aanbevolen wordt hierin te investeren. Voor verkeersveiligheidsbeleid en -onderzoek is het van belang om over de in BRON ontbrekende verkeersdoden meer informatie te hebben dan nu beschikbaar is. De SWOV ontvangt van het CBS alleen uitsplitsingen naar steeds één variabele, terwijl meer (geanonimiseerde) informatie op slachtofferniveau gewenst is. Aanbevolen wordt om na te gaan of het binnen de privacywetgeving die geldt voor het CBS toch mogelijk is dergelijke informatie beschikbaar te stellen voor beleidsvorming en onderzoek. Wij bevelen aan om onderzoek te doen naar hoeveel verkeersdoden er gemist worden door onjuiste bepaling van de doodsoorzaak bij artsen en instellingen. In dat onderzoek zou er met name naar gekeken moeten worden of er specifieke risicogroepen zijn die stelselmatig meer onjuist geregistreerd worden dan anderen. Ook zou er moeten worden onderzocht of dit proces verandert in de tijd. Ten slotte bevelen we aan dat de SWOV samen met het CBS het onderzoek naar de toegevoegde waarde van de LMR ook uitvoert voor 2010 zodra de LMR voor 2010 beschikbaar komt.
The registration of road fatalities in the Netherlands; Description and assessment of the registration process In order to be able to develop policy for the reduction of road safety hazards, it should be known how big the road safety problem is and where the biggest problems occur. Information about the number of road crashes and casualties is therefore very important. Even more important, however, is information about the characteristics of the crashes and casualties. The most relevant characteristics are to be derived from the Database Registered Crashes in the Netherlands (BRON). This database is compiled on the basis of data collected by the police and presented to the Centre for Transport and Navigation (DVS) of the Directorate-General for Public Works and Water Management. However, not all crashes are registered in BRON. Nevertheless, to get a proper impression of the road safety situation in the Netherlands, two important indicators, the number of road fatalities and the number of serious road injuries, are not determined on the basis of BRON alone. The number of serious road injuries are estimated by SWOV annually. For this, the Dutch National Medical Registration (LMR) is used in addition to BRON. Since 1996 the number of road fatalities in the Netherlands has been determined by Statistics Netherlands annually, in cooperation with the Ministry of Infrastructure and the Environment (IenM). This occurs on the basis of three sources: the so-called B-records (cause-of-death records that in principle are sent to Statistics Netherlands in the case of any death), records of the district prosecutor’s office and BRON. Reason for and objective of the study On 18 April 2011 the Minister of Infrastructure and the Environment published the number of road fatalities in 2010 as determined by Statistics Netherlands: 640. This shows that the number of road fatalities was reduced by 11% in comparison with 2009. At the same time, the registration rate of road fatalities in BRON dropped to 84%, which is much lower than in previous years. This was reason for SWOV to study the quality of the registration of road fatalities in the Netherlands in greater depth. First of all, the objective of the study was to discover what the reasons are for the reduction of the registration rate of BRON in 2010. Next, the aim was to determine whether it is plausible that road fatalities are overlooked, in other words, whether the number of fatalities in the Netherlands is in actual fact different from what Statistics Netherlands has determined. Findings with respect to the usefulness of police registration for BRON An important development in 2010 was that the so-called registration set for police crash registration was abolished. This form always included extensive information about crashes. Since the registration sets have been dispensed with, the police more often use characteristics reports that, however, do not include the information that is needed to serve as a basis for road safety policy or research. Another important development was the implementation in 2009 of one overall national information system (Basic Enforcement Facility or BVH) for the police for the registration of reports, the processing of reporting and finalizing incidents. In 2010 the Netherlands Court of Audit drew the bleak conclusion that the implementation of BVH could have a negative effect on the quality of road crashes registration. It was not possible to prove that the two developments mentioned above are the reason for the sudden reduction in the number of road fatalities registered in BRON. However, sufficient indications have been found in this survey to conclude that such a correlation does exist. It must be observed, however, that in the Netherlands the police are informed of every death of unnatural causes. This means that the road fatalities that are missing in BRON are known to the police, not necessarily, however, as road crash casualties. Data concerning these fatalities is therefore not sent to the Centre for Transport and Navigation. Findings with respect to the number of road fatalities The Statistics Netherlands road fatalities statistics are based on BRON only to a limited degree. The other two sources that road fatalities statistics are based on (the B-records and the records of the district prosecutor’s office) are independent of BRON. The reduction of the registration rate of fatalities in BRON to 84% need therefore not have consequences for the registration of road fatalities in one of the other two sources used. When a road fatality has been registered in at least one of the three sources, it will be included in the Statistics Netherlands road fatalities statistics. On the basis of the information provided by Statistics Netherlands, we conclude that, on the basis of the available data, Statistics Netherlands correctly determines the number of road fatalities annually occurring in the Netherlands. With respect to the completeness of the data that Statistics Netherlands relies upon, we conclude that it is quite likely that certain road fatalities are considered as natural deaths, whereas in actual fact they are the result of road crashes. In our view, this underregistration is the responsibility of the medical practitioner in attendance who has to determine whether or not someone has died of natural causes. We know from the literature that things go wrong during this stage of the registration process. It cannot be determined without further research how many road fatalities are overlooked, or whether this concerns a specific subgroup of road fatalities. For as yet, it is not known whether this occurs more often, less often or equally often compared to a number of years ago. When determining the number of road fatalities, Statistics Netherlands does not (yet) make use of LMR. However, this database includes data of patients discharged from Dutch hospitals (death being a type of discharge) and can therefore be an addition to the three sources already used by Statistics Netherlands. The following issues should hereby be taken into account: - In LMR death need not be the result of the road crash mentioned. - With road crashes in which no motor vehicles are involved, LMR does not require that this should have taken place in the public road. Road crashes in LMR are therefore not necessarily road crashes according to the formal definition. In order to study in what way LMR, despite these disadvantages, can have added value in determining the cause of death, the LMR records of the period 2007-2009 have been analyzed; unfortunately, at the time of this study LMR was not yet available for 2010. On a yearly basis these records include circa 65 deaths as a result of road crashes that can however not be found in BRON. At the request of SWOV, Statistics Netherlands examined whether these persons are included in their road fatalities statistics. Statistics Netherlands found that circa half of these deaths were present in the road fatalities statistics. Statistics Netherlands came to the conclusion that in the other half (circa 35 deaths) another cause of death was concerned. For the major part of these deceased, they found indications in the remaining sources that death had been due to a road crash. Of five deaths annually recorded by LMR as road crashes, Statistics Netherlands found no indications in the other sources that these deaths were not related to road crashes. These deceased may therefore possibly be overlooked when determining the real number of road fatalities. Since the LMR data for 2010 was not yet available at the time of this study, it was not possible to investigate whether the above-mentioned findings also apply to 2010. In other words, it is not yet known whether a maximum of five deceased persons were also overlooked in 2010. Recommendations It is not easy to understand why the registration rate of road fatalities in BRON could not be 100%. Crashes involving very serious injuries and fatalities are very serious incidents about which the police are or should be informed. The additional administrative load of making reports or registrations sets of road crashes by the police can be expected to be limited, certainly in the case of fatalities. The police are actually already informed about them, because they are informed of each non-natural death. It is recommended that these casualties be all registered in BRON. SWOV advices to consider the reintroduction of the registration set for road fatalities and seriously injured. Another recommendation is to add more variables to the characteristics report, so that it is simpler to link it to other information from other sources. The necessary improvement of the registration by the police implies that the police should first of all be given new indications. Next, the police organization should be given the opportunity to meet the agreements. This involves among other things that the software used by the organization must become user-friendlier. This results in greater efficiency and higher quality. It is recommended to invest in this. It is in the interest of road safety policy and research to receive more information about the road fatalities missing from BRON than is available right now. SWOV only receives from Statistics Netherlands itemizations of one variable at the time, whereas more (anonymized) information on casualty level is desirable. It is recommended to investigate whether in the context of the privacy law it is nevertheless possible to make this kind of information available for developing policy and research. We recommend investigating how many road fatalities are overlooked as a result of medical practitioners and organizations inaccurately determining the cause of death. This study should mainly investigate whether specific high-risk groups are systematically registered inaccurately compared to other groups. It should also be studied whether this process changes over time. Finally, we recommend that SWOV, in cooperation with Statistics Netherlands, will also study the added value of LMR for 2010, as soon as LMR is made available for 2010.
