Abstract
A police form is discussed to be used when contravening art. 26 of the "Wegenverkeerswet" (Dutch Traffic Regulations). Both the police and the doctor have to fill up a copy. A score is given for clearly perceptible phenomena, which are also understandable for non medically trained personnel. It is decides to use this form in Rotterdam.
