Pilot factor : a problem in aviation safety.

Abstract

If we want to prevent aviation accidents, we must investigate all aspects and determine root causes, including such causes that may have been contributed by the management of the organization. Repeat cause factors are evidence of management failure. The safety system is well formulated and will work, if utilized properly.
