Abstract

The theme of this paper is that one must establish root causes in order to set in place effective preventive measures for incidents. Whilst this would appear to be a straightforward matter at first sight, in practice there are many pitfalls. The aim of this paper is to map out some of the key problems and offer alternative approaches for root cause analysis. (A)
