Abstract

Human error, a significant contributing factor in a very high proportion of civil transport, general aviatation, and rotorcraft accidents is investigated. Correction of the sources of human error requires that one attempt to reconstruct underlying and contributing causes of error from the circumstantial causes cited in official investigative reports.
