By the lack of a uniform vision on the aims of the first aid in ambulances the test to its quality is not well possible. The bottlenecks in the current situation in the Netherlands are described. When advanced medical treatments should be a part of such care then the requirements for the personnel and the emergency vehicles should be adapted.
