An approach to systematic learning from accidents.

Present methods in ship accident analysis and documentation have a number of shortcomings which prelude a systematic and efficient increase in safety standards. This is especially true when dealing with human and organisational error (HOE). This situation is not tolerable in the long run as long as it is still known that HOE factors are the dominant cause in the majority of accidents and even in the most catastrophic ones. This paper proposes a method for accident analysis that better reflects the characteristics of ship accidents. (A)
