The objective of this report is: to make a full review regarding the number of bicycles in China, their role in transport, bicycle accidents, epidemiology of bicycle trauma and its characteristics of injury severity, and to put forth relevant suggestions for how to decrease bicycle trauma; to collect and analyze representative papers. The results show that: (1) China is the "kingdom of bicycles" in the world; (2) bicycles are still one of the important transport tools in China. The population of people who use bicycles as a transport tool accounts for over 30% in most cities in China; (3) about 70% of all traffic accidents in China are related to bicycles, mainly due to vehicle-bicycle collision; (4) the incidence and epidemiologic characteristics of bicycle injuries vary greatly in different regions just due to factors such as terrain, number of bicycles, road conditions and weather. The majority of the casualties are young people; (5) the wounding mechanisms and characteristics of bicycle injury severity vary with different collision types. The injury severity due to bicycle-vehicle collision is the most severe. Bicycles, although having brought a lot of advantages to humans, could make some troubles due to its increasing number. Therefore, bicycle injury research should be strengthened.
