Bicycle spoke injuries of the lower extremity are a frequently encountered accident in the young child. The resultant morbidity is significant. Twenty-two of 27 children developed full-thickness skin loss about the foot and ankle. Ten of the 22 required split-thickness skin grafts for coverage of their wounds. The average hospital stay for those requiring grafts was 24 days. The mechanism of injury is similar to the crush sustained in wringer injuries of the upper extremity.
