Past analytic and prevention efforts have not approached pilot-error accidents in the context of malfunctions among the basic man-machine-environment elements. Such an approach was proposed and seeks to identify the common human-error events in pilot-error accidents. A factor analysis produced nine distinct, meaningful and representative helicopter and airplane factors. A component score analysis yielded pilot and mishap background information which was helpful in interpreting the factors. An experimental human-error events reporting form was developed which holds promise for clearer identification of mishap-causing system elements and corrective measures required.
