Tri-level study of the causes of traffic accidents. Volume II: Appendices.

This paper summarizes procedures and techniques developed in conjunction with a tri-level accident investigation program. The three levels of the tri-level program are described, including the principal data collection forms and procedures. Using this methodology, accident causes were identified and ranked.
